Silent failures in AI automation: evidence before success
How to detect automations that finish without an error but miss the outcome, using postconditions, external evidence and fail-closed boundaries.
The most dangerous error does not always throw. An automation can return a zero exit code, produce a convincing message and still fail to update the right resource, preserve required data or satisfy the final condition.
Success needs a postcondition
Before execution, write down what can be observed afterwards:
- the expected resource exists and has the correct identity;
- remote state confirms the change;
- files outside the scope remain untouched;
- a repeated attempt does not duplicate or corrupt the result;
- logs and artefacts allow the operation to be reconstructed.
“The command finished” is evidence of execution, not evidence of outcome.
Fail closed at boundaries
Ambiguous input, insufficient permission and redirected destinations should stop an operation before its first write. This discipline guided the OmniCodex installer regression: symlinks in the manifest path are now rejected before a backup or profile installation can occur.
The change reduces one specific class of unintended write. It does not solve race conditions, rollback or whole-installation atomicity.
Test the effect that must not happen
A good regression test checks more than an error message. It prepares an external destination, triggers the dangerous condition and proves that nothing changed. This negative evidence is essential for installers, publishers and repository tooling.

This is an independent analysis published by MoroniGL.dev, also informed by OmniCodex PR #5. The audit consulted the free Silent-Failure Checklist description from Flux Growth Agency. Because a conventional redistribution licence was not verified for that material, this article does not reproduce its list or wording.
